The Pharmacy Clinical Specialist coordinates medication use, related education, and research to provide pharmaceutical care to patients.
Manages drug therapy protocols as a part of an interdisciplinary team.
Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug-related problems.
Provides drug information and education to medical staff and patients.
Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
Educates trains and precepts pharmacy students and/or pharmacy residents.
May perform job responsibilities of Pharmacist Staff.
Provides drug information and education to patient care staff.
Licensed Pharmacist credentialed from the Michigan Department of Licensing and Regulatory Affairs (LARA) obtained prior to hire date or job transfer date.
Controlled Substance License required.
Board certification in area of specialty preferred.
Bachelor of Science degree with Board Certification and/or relevant practice experience required or Pharm D required OR clinically oriented Master of Science degree in Pharmacy required with PharmD preferred
A residency in the provision of clinical pharmacy service or 2 years of experience preferred.
