For this role it is preferred that the candidates have experience with Cerner, at least 3 years experience focused on Oncology patients or BCOP certification and acute care experience and/or Ambulatory care experience
The purpose of this position is to provide oncology pharmacy the services to patients in our cancer care center which includes the responsibility for the safe and proper distribution and use of oncologic medications in acute care patients as well as ambulatory patients. This pharmacist will also provide pharmaceutical care to non-oncologic patients as determined by departmental needs.This position is to function at higher decision making and accountability levels regarding medication therapy, proactively affecting medication use by making recommendations at the time prescribing decisions are being made or, in some cases, making independent decisions about medication therapy in cooperation with other healthcare team members. Additional emphasis will be placed on provision of services across the continuum of care. Expected activities include:o Coordinating the complete medication plan from inpatient chemotherapy regimens to home medications including oral chemotherapy and supportive medicationso Participate in patient/family education on what to expect during treatment and post treatmento Conduct post discharge counseling callso Compliance with USP 800 processes The pharmacist must possess strong communication skills for patient education and work collaboratively with inpatient and outpatient interdisciplinary teams. These include but are not limited to providers, nurses, case workers, pharmacists, and technicians. There is also the expectation of a teaching and precepting component of students, interns and residents. Strongly desired attributes include:
- Experience with Cerner® EHR
o Identify and participate in medication Use evaluations and oncology stewardship activities.
o Educate members of the healthcare team on medication use and adherence to guidelines.
o Provide discharge counseling, and oral chemotherapy counseling as well as answer questions relating to use of integrative medicine
o Review patient medication profile review for appropriateness, clinically relevant drug interactions and make pharmacogenomics recommendations as appropriate.
- Recommended to be BCPS credentialed within 3 years of hire
Registered with Missouri Board of Pharmacy as Pharmacist or registry eligible and obtain within 90 days of hire
**Eligibility for licensure requires submission of application for Pharmacy License at time of hire**
