The Clinical Pharmacist will be responsible for performing patient assessments; creating care plans; medication reconciliation; lead patients’ medication therapy management teams by counseling patients on critical factors that include: dosage, duration, conditions, adverse events, drug interactions, monitoring parameters; recommends formulary alternatives when appropriate; compounds and dispenses medications when necessary; and participates in approved collaborative practice agreements and protocol, and more.
Bachelor of Science degree in Pharmacy from an approved college of Pharmacy required. Doctor of Pharmacy (PharmD) degree preferred.
Registered as a licensed pharmacist or eligible for reciprocity in the State of TX. (Must obtain a registered license within 3 months of employment.)
Excellent communication and interpersonal skills and experience working on interdisciplinary initiatives across all levels within the pharmacy
2-3 years’ experience directly related to the position
Sterile Compounding Certification preferred
Ability to work independently, set priorities, make critical decisions, and respond to client concerns
