Ambulatory Care Clinical Pharmacist

The Ambulatory Clinical Care Pharmacist is responsible for the development, design and delivery of direct patient care services in conjunction with the Student Operated Pharmacies at the College Hometown Pharmacy within the Hometown Health Center in Schenectady and College Parkside Pharmacy, in the Capital South Campus Center in Albany. Direct care includes the provisions of medication therapy management, point of care screening, immunization delivery and other health care focused clinical initiatives. The Ambulatory Care Clinical Pharmacist will also precept student pharmacists on introductory and advanced pharmacy practice experiences. Develops policies, standards and protocols to enable efficient delivery of direct patient care services consistent with the School’s vision, mission and strategic plan while meeting all state and federal regulatory agencies’ requirements. The Ambulatory Care Clinical Pharmacist will interface with the School of Pharmacy in developing curricular initiatives around the Student Operated Pharmacies and will participate in the delivery of these services and the training of others to do the same.
