Position Summary

The Clinical Pharmacist Specialist per diem is responsible for improving therapeutic outcomes for our patients through safe, rational and cost-effective pharmaceutical care, education and research.  The Clinical Pharmacist Specialist per diem maintains competency in drug therapy and disease-state management in order to provide this care and improve patient care outcomes.  The Clinical Pharmacist Specialist per diem provides drug information to physicians, nurses and other health care workers to improve the quality of medication prescribing and administration.  The Clinical Pharmacist Specialist per diem assists with medication reconciliation, identifies potential medication safety issues and works actively to prevent medication errors. The Clinical Pharmacist Specialist per diem educates patients and families to improve compliance with drug therapy as required.  The Clinical Pharmacist per diem Specialist directs the work of ancillary personnel as necessary.

Essential Functions and Responsibilities of the Job

• The Pharmacist Specialist per diem reviews prescriptions considering dosage, current therapy, potential drug interactions, and patient diagnoses to ensure the safe use of medications by patients.  When changes in therapy are necessary, the pharmacist effectively communicates the need for change with the patients’ prescribers.
• The Pharmacist Specialist per diem provides clear and concise information to patients about the medications they are taking to allow patients to safely self-administer their medications.
• The Pharmacist Specialist per diem evaluates patients’ insurance status to ensure that the charge for the medication dispensed or the service provided is appropriate and the reimbursement received from payors meets expectations.
• The Pharmacist Specialist per diem directs the daily activities of ancillary pharmacy personnel and works collaboratively with other pharmacists to optimize the productivity of the pharmacy.
• The Pharmacist Specialist per diem works with the Intake/Authorization staff and insurance companies to obtain Prior Authorizations, communicates with physicians and patients about changes in medication coverage.
• Promotes patient centered customer service.
• The Pharmacist Specialist directs and oversees the adherence to regulatory requirements and the QA programs.
• Performs other duties as assigned.
