The NHMG Clinical Pharmacist (CP) will be based within the Retail & Specialty Pharmacy (RSP) department and report to the RSP Clinical Manager. The NHMG CP will function as an integrated team member within Wilmington-based NHMG provider practices by providing direct patient care and pharmacy services consistent with primary care practice. The NHMG CP will assess, develop, plan and evaluate individual care plans that promote optimal health while developing personalized relationships with patients and their support systems. Our pharmacy team exists to ensure safe, effective and affordable access to medications for our community. The NHMG CP will also work collaboratively with NH NHRMC system pharmacies to further our integrated pharmacy patient care model to reach our goal of optimal patient care. The NHMG CP will also have the opportunity to work with team members in support of population health initiatives for NH NHRMC sponsored plans. Preferred Experience
Providing patient care as Clinical Pharmacist Practitioner (CPP) or equivalent
Direct patient care experience of 1-2 years in the primary care practice setting
Functioning as team member within health system pharmacy integrated care delivery model
Provision of care through telehealth platforms
MTM & stars based outcomes
Preferred Credentials
PGY2 Ambulatory residency
Current BCPS or BCACP (board certification will be required post-employment for MSO credentialing)
Current NC Pharmacist license or reciprocity within 30 days of start
NC Board of Pharmacy Clinical Pharmacist Practitioner registration required for this role once hired
