This is an internal job announcement. Current permanent VA employees only. The incumbent is under the supervision of the Anticoagulation Supervisor, or designee. The incumbent performs clinical, administrative and management duties for the Anticoagulation Clinic and Clinical Pharmacy Specialists. These duties include, but are not limited to, telephone management, scheduling support, secure messaging coordination and support, patient laboratory value monitoring, medication utilization monitoring, and supplemental patient education communication. Duties include but are not limited to: Clinical Functions: Displays ability to learn clinical tasks and operate within the scope of those functions, under the direction of a pharmacist. - Conduct patient interviews, including medication history, anticoagulation history, and effectiveness of anticoagulation, need for medication refills/renewals, and investigating causes of therapeutic irregularities. Independently gather, evaluate, and analyze data of patients as per established protocols. Routinely review computer medication profile for prescription duplication, therapeutic duplication, prescription non-compliance, and allergies or adverse drug reactions. Reports identified patients to assigned Anticoagulation Specialist for evaluation. Routinely review warfarin and direct oral anticoagulant (DOAC) dashboards and contacts patients who have not returned for laboratory monitoring. Provides population management functions using patient care dashboards to identify patients for potential intervention. Document all patient specific clinical decisions (in consultation with the clinical pharmacist) in the patient's electronic record. Administrative: Responsible administrative activities for the clinic related to faxes, phone management and coordination of secure messaging activities. - Assists the AMSA with managing the clinic faxes, reviewing patient information, triaging and facilitation appropriate action from clinical pharmacy specialists. Assists the AMSA with providing telephone management, triaging phone calls and triaging and facilitation appropriate action from clinical pharmacy specialists. Evaluate, analyze workflow, and coordinate work activities to continually improve the efficiency and safety of Clinical Pharmacy practices. This will include involvement with formal and informal improvement activities. Coordination of care for off-station laboratory monitoring and travel requests. Training/Education: Comprehensive knowledge of medical center and pharmacy policies and procedures sufficient to train and orient new employees and other staff. Coordinate and assists training of new employees as assigned. Involvement with pharmacy management to assist with development and updating of policies. Utilize VHA office systems to develop and share education, specifically through use but not limited to the following: Word, Excel, Outlook, and SharePoint. Performance Improvement, Quality Assurance and Systems Redesign: Responsible for issues pertaining to continuing quality improvement, quality assurance and regulatory issues. Evaluate, analyze workflow, and coordinate work activities in order to continually improve the efficiency and safety of the Clinic. This will include involvement with formal and informal improvement activities. Identify, initiate, and document cost savings and patient safety initiatives. Independently gathers, evaluates, and analyzes data to generate workload reports and/or for other projects/statistical purposes. Customer Service and Teamwork: Maintains excellent working relationship with Clinical Pharmacy Specialists, Pharmacy staff, and providers. Is courteous and professional at all times. Effectively promotes teamwork among staff, residents and students. Provides prompt and personal attention to each patient.
