Clinical Pharmacy Specialist
The Anticoagulation Clinical Pharmacy Specialist (CPS) is responsible for all aspects of pharmaceutical care for Veterans in the anticoagulation clinic at the Minneapolis VA. The CPS is a licensed pharmacist that has knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics for anticoagulation therapy. Major Duties: The anticoagulation CPS provides appropriate selection of anticoagulant therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
