The incumbent of this position as the Clinical Pharmacist Specialist responsible for the full range of professional, clinical, administrative and technical functions of the clinical pharmacy program in Huntington and Community Based Outreach Clinics Major duties are below but are not limited to the following: Provides direct patient care (minimum 25% of the time) in primary care clinics requiring a scope of practice be maintained including the following: Anticoagulation Service Pharmacotherapy Review Services (including but not limited to Congestive Heart Failure (CHF) Service, Cardiovascular Risk Reduction Services, counseling patients (inpatient and outpatient) who have these disease states at respective sites. Serves as a consultant and information resource to primary care at the medical center. Develops, implements, and manages programs that maximize patient outcomes and decrease drug therapy costs (i.e., Drug Utilization Evaluations, participation in the development of disease management programs). Works closely with the Chief, Pharmacy Service to ensure quality, cost effective care in the medical center. Serves as a consultant and information resource for physicians and healthcare staff throughout the medical center. Provides patient education. Supervises and precepts pharmacy residents and students during ambulatory care and geriatrics rotations as necessary from affiliated schools of pharmacy. Provides lectures and other educational activities for professional staff as needed. Publishes articles of interest in peer reviewed journals as the opportunity arises. Screens and completes all prescriptions generated for potential problems and will resolve questions with provider. Documents and dispenses limited quantities of acute care medications (i.e., antiinfectives, antihypertensives). Maintain appropriate inventory to meet the needs of the clinic. Completes monthly inspections of respective areas to ensure all legal and Joint Commission requirements are met. Work Schedule: Monday - Friday 8:00am-4:30pm Financial Disclosure Report: Not required
