The Employee is responsible for the daily professional and administrative operation of the clinical pharmacy and educational programs. Pharmacy Service at VA Illiana provides innovative and progressive clinical services in specialty and general patient care areas that foster and support the provision of pharmaceutical care. Duties: -Designing, developing, implementing and assessing progressive and comprehensive clinical pharmacy services. -Responsible for the oversight of the education of clinical Introductory Pharmacy Practice Experience (IPPE) and Advanced Pharmacy Practice Experience (APPE) students in all area(s) of practice. -Responsible for serving as Clinical Pharmacy Specialist and providing clinical services for institutional and ambulatory services within area of clinical practice expertise. -Develops, coordinates, supervises, evaluates, and assumes primary responsibility for all clinical pharmacy programs and services in order to maintain a superior quality of services offered by pharmacy personnel. -Instructs, precepts, and supervises pharmacy residents, students, and pharmacists in order to expand their capabilities in providing and monitoring rational drug therapy. -Face-to-face comprehensive medication management of patients within area of clinical practice experience. -Virtual care modality visits, such as: Veteran requests through secure messaging, telephone-based care, VA Video Connect, and clinical video telehealth. -Functions independently under scope of practice as defined by the health care system for selected disease states to provide pharmaceutical care to patients. This includes but is not limited to the design, implementation, and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers. -Counsels patients regarding their medication(s) in accordance to federal and state requirements, VA standards, and pharmacy best practices. This would include oral counseling and providing written information. Provides documentation of all counseling in appropriate records. -Teaches staff and patients on a wide range of medical subjects. Teaching may be in regularly scheduled classes or may include presenting lectures, preparing hand-outs, giving demonstrations, newsletters, or other means available. -Is responsible for the education of PGY1 pharmacy residents in respective area(s) of practice and maintaining residency preceptor requirements according to the ASHP Accreditation Standard for PGY1 Pharmacy Residency Programs. -Completes peer reviews in an accurate and timely manner.
