The VA Sierra Nevada Health Care System is currently recruiting for a full time Clinical Pharmacy Specialist (Home in Hospital Program) position in the Pharmacy Service, located at the Reno Division. The Hospital in Home (HIH) Clinical Pharmacy Specialist (CPS) at VA Sierra Nevada Health Care System (VASNHCS) functions at the highest level of clinical practice and works under a scope of practice as approved by the Executive Committee of the Medical Staff (ECMS) and facility collaborative care agreement. The CPS functions to initiate, modify, discontinue, and monitor medication therapy and acts as a consultant for comprehensive medication therapy management services. This includes, but is not limited to: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. The HIH CPS will perform their duties for patients enrolled in the HIH program and improve health care through optimal drug therapy. Optimize the time of other healthcare providers on the Hospital in Home Team, Home Based Primary Care Team (HBPC), Emergency Department, Internal Medicine, Patient Aligned Care Teams (PACT), and specialty clinics by performing at the top of licensure under a Scope of Practice. The HIH CPS will document positive patient outcomes and cost-effective interventions. Serve as the pharmacy expert for the Hospital in Home program, complete patient profile reviews, attend daily Hospital in Home team rounds and manage both chronic and acute care disease states. Work Schedule: Monday-Friday, 8:00am-4:30pm with holiday and weekend rotation. Tour of duty may vary depending on staffing needs. Financial Disclosure Report: Not required
