Clinical Pharmacist Specialist (PC-POP)
The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients. This CPS functions as a member of the Primary Care Pain Education and Opioid Monitoring Program (PC-POP), serving as a mid-level provider to initiate, modify or discontinue medication therapy for face-to-face, telehealth, or telephone appointments. Essential Functions: Conduct patient interviews and assess (to include, but not limited to), medication therapy and adherence, adverse drug reactions and drug interactions, and medication reconciliation.
