The VA Northeast Ohio Healthcare System is recruiting for a Pharmacy Technician. The Pharmacy Technician will function in Pharmacy Service. The duties of the Pharmacy Technician include, but are not limited to: Anticoagulation patient preparation, stocking of clinic supplies, and monitoring of clinic flow to ensure timely patient visits. Obtain and assess information on compliance, current warfarin dosage, medication changes, alcohol consumption, smoking, bleeding symptoms and general state of health. Initiate electronic progress note in CPRS, including assessment for warfarin indication, duration, and INR range. Reviews medication changes, adherence, and complications of therapy and report findings to a Clinical Pharmacy Specialist. Monitors laboratory data, including, but not limited to stool guaiac tests and biannual CBC. Evaluate INR stability and assesses etiology of INR's out of range. Perform point of care testing and report results to CPS or other designated clinic staff as appropriate. Maintain point of care equipment and perform quality controls as necessary. Complete electronic progress notes, schedule appointments, complete consults, and document data in VISTA/CPRS. Answer telephone, voicemail, and emergency cellphone to triage calls. Process prescriptions through the computer and tracking through CMOP. Enter progress notes into the patient medical record. Enter orders for lab tests. Work Schedule: Monday - Friday, 8:00am - 4:30pm, with rotating weekends and holidays Financial Disclosure Report: Not required
