Pharmacist (Clinical Pharmacy Specialist)
The Mental Health (MH) Community Based Outpatient Clinics (CBOC) Telehealth Clinical Pharmacy Specialist (CPS) is a licensed pharmacist with mental health experience. Includes taking responsibility for outcomes associated with medication use, referral of requests to the appropriate level of care, process improvement, and functioning at the highest level of clinical practice by working independently per their scope of practice as a mid-level provider to perform comprehensive pharmaceutical care. Major Duties: Mental Health Program Services: The MH CBOC Telehealth CPS will participate in a patient-focused, team-based multi-disciplinary model of patient centered care to promote positive patient outcomes through direct and indirect interaction with patients and providers.
