Clinical Pharmacist Specialist, Anticoagulation
The Clinical Pharmacy Specialist, Anticoagulation reports directly to the Associate Chief, Clinical Pharmacy Services, or their designee. Incumbent will support the mission of the Department of Pharmacy Services and the Louis A. Johnson VA Medical Center. The incumbent is also accountable to the Chief, Pharmacy Service, or their designee, for professional licensure, competency, and continuing education. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients. A CPS functions as a mid-level provider to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services that includes improving and promoting bes
